Registration forms may be down loaded below or picked up at The Boys and Girls Club,

  201 Dale Scott Ave.

 OCYSL Fall Registration 2010

 

Early Registration Discount of $5 off the $45 fee if you  registration in  person at the shed on July 31, August 7, 2010 and August 21, 2010 is the last day of registration.  Players MUST have turned 4 prior to July 31, 2010

 

These are the  in person registration dates and times

July 31st @ Mountain View Soccer Shed 10am - 2 pm

August 7th @ Mountain View Soccer Shed 10 am - 2 pm

August 14th @ Mountain View Soccer Shed 10 am  2 pm

August 21th @ Mountain View Soccer Shed 10 am  2 pm

 

All returning players from the Fall season will automatically return to the team they played on in the Fall.

  

Registration Fees

(as of July 31, 2010)

All players -- $45.00

Fall Season Registration Fee Sibling Discounts:

1st Child pays full fee of $45.00

2nd Child: $5.00 discount, pays fee of $40.00

3rd Child: $10.00 discount, pays fee of $35.00

4th Child: $15.00 discount, pays fee of $30.00

 

How To Register

♦ Fill out the attached Registration Form completely and accurately. Remember to put what practice days you prefer.  We will do our best to accommodate your requests but can not guarantee that all your requests will be met.

♦ Attach a small (about 1 x 1) photograph of the player to the Registration Formfor Fall Season players only.

Pick Up League does not require photos. You can also mail in registration to the adders below.

♦ Purchase your players uniform at the OCYSL Shed

Located on Santa Cruz Avenue (next to Mountain View Middle School) on the following days:

July 31, August 7, August 14, and August 21, 2010  

Uniform Set (1 Blue Jersey, 1 Silver Jersey, Shorts, Socks) $35.00

Jersey Set (1 Blue Jersey, 1 Silver Jersey) $25.00

Shorts $10.00

Socks $3.00

♦ Wait for a call or email from your players coach to let you know what times and where your player will be

practicing. Double check your players Registration Form to make sure you have provided a current phone number

and email address!!

Go to www.ocysl.net and watch for the team Practice Schedules and other important information. 

 

Are you interested in having your child play soccer for free?????!!!!! Volunteer with OCYSL!

Head Coaches: if you volunteer as a team head coach for the OCYSL, all of your children will pay no registration fees

for the seasons you are coaching

Board Members: if you become a OCYSL board member, all of your children will pay no registration fees for the

seasons you are a board member

Before you turn in your application, please add to the bottom of the from, if you are a returning player, what number you have, if you have a friend you want to be on the same team with, a coach you want to play for, or a set of days you want to practice, Mon/Wed, or Tue/ Thurs.

  Refund Policie: You will get a full refund before practices start, 1/2 refund after practices start but before games start and NO refund after midnight before the first game.
